Guidelines for grant applicants

Who can apply?

Applications will be welcomed from or on behalf of any child or young person up to age 26 suffering from any condition physical or mental that requires a mobility aid or other equipment that facilitates independence, where these aids are not normally available from a statutory body and are subject to acceptance of the Conditions of Loan document hereto attached.

If applicant is under the age of 18 a parent or legal guardian must complete the application form on their behalf.

The value of grants given will depend on an assessment of all the relevant information made available to the Trust. Please, therefore, complete the application form in full, do not leave blank spaces.

What can you apply for?

Wheelchairs

Applications may be considered towards the cost of outdoor powered wheelchairs where these are not provided by statutory authorities. Wheelchairs would normally remain the property of AFKCT and be provided on a permanent loan basis to the recipient. The recipient of the wheelchair or guardian of the recipient must take responsibility for the insurance of the wheelchair in respect of third party liability, the Trust will be responsible for the maintenance of the wheelchair. Specialist assessments will be required.

Specialised Mobility Equipment

Grants may be given to assist in the purchase of other mobility equipment for specific purposes. These purposes may be educational, recreational or vocational but must be clearly defined in the application. Specialist assessments will be required.

Therapeutic Equipment

Applications may be considered for therapeutic equipment, specialist assessments will be required.

Communication Aids

Specialist assessments will be required.

How applications can be made

Applications can only be made by completing the attached form. Any professional assessments, supporting statements / letters, quotations, and a recent photograph must be attached to the application form.

Funding policies

The Trust will first agree an application and then fund for the applicant's equipment, normally in its entirety, but reserves the right for a contribution to be made toward the cost of items where deemed necessary.

The Trust will not normally consider applications for items of equipment in those cases where there is a duty on the statutory authorities to provide them.

The Trust will not normally consider applications where it is deemed that the applicant or applicants family are capable of providing the equipment from their own resources.

Assessments

Under some circumstances, as noted in the guidelines, the Trust will require professional assessments. The Trust also reserves the right to require professional assessments for other items at its discretion and the Trust may need to seek additional information on occasions.

Please note a clinical appraisal form must be completed by a health professional at time of assessment.
